Maybe, the option to extend must be requested in advance. Whether due to travel, other appointments or energy level, the option to extend past the initially booked time allotment isn't always possible. So make sure to book for longer than you think you'll need or ask in advance if the option to extend is available.
Depends on your provider. Some service providers have a menu of services and will offer a limited number to choose from. Some offer the Girlfriend Experience, meaning what ever unfolds between you during your booking is included in the rate.
Some service providers require a deposit/reservation fee before they will meet you. This due to the extraordinary amount of time wasters (ppl who book and then ghost.) A deposit may only be required the very first time or may be required every time, ask your service provider about their deposit preference.
The service provider must still prepare (energy, hygiene, beauty) and travel the same amount if you book a whole hour or just 30 min, so if the service provider offers a 30min booking, that is taken into account.
